Medical treatment consent
I give permission for ARISE Church staff and volunteers to obtain emergency medical, hospital or ambulance assistance at anytime they consider necessary. I understand that every effort will be made for me or my emergency contact listed above to be notified before instituting such procedures. I acknowledge that I will be liable for any and all medical/hospital/ambulance costs and expenses incurred in my treatment.
I also understand that while every reasonable precaution will be taken to ensure my practical protection, I hereby release and hold harmless ARISE authorised staff and volunteers from any and all liability in the event of any injury, accident or misfortune, damage or loss that may occur to me and my property while present at the ARISE Church Event.
Media waiver
I understand that no personal recordings (audio or visual) of the conference may be taken.
General
Your registration and payment are confirmation that you have read and accepted these terms and conditions
Disclaimer
The conference will be captured in photographs, audio and visual. ARISE Church reserves the right to use the material for promotional purposes. ARISE Church recommends that you confirm your registration with our office prior to booking any flights and/or accommodation. ARISE Church does not accept any responsibility for losses incurred for flights and/or accommodation booked for the conference. ARISE Church reserves the right to change any of the published conference details without notice and have the final say on any matter relating to the conference.
Refunds
There will be no refunds. However you may request to transfer your registration to another person prior to the 2nd August 2012 to ARISE Church in writing, transfers will incur a $15 administration fee. Delegates who register within 10 days of the conference will need to collect their confirmation letter when they arrive at conference.
